We ensure that all personal data, including sensitive personal information, which you will provide or that we will collect, is kept secure and confidential. 

This Privacy Notice explains how we collect, use, process, secure, and disclose personal data obtained for medical consultations and other related clinical services. We may update this notice from time to time to reflect changes in law or our internal standards. Please review this privacy page periodically for the latest version. 

By accessing HealthFirst Clinic services and submitting your personal data, you acknowledge that you have read and understood this Privacy Notice and consent to:

  • The processing of your personal data to provide the intended services, in accordance with applicable laws, regulations, and our policies, including sharing with subsidiaries, affiliates, contractors, service providers, and other authorized third parties for legitimate purposes. 

 

  • Making your personal data available to our parent company, subsidiaries and affiliates to ensure we provide or recommend the best solutions for your health needs.

The Personal Data requested, as appropriate, may include: 

  • Patient's Name (Full Name, Middle Name, Last Name) 

  • Patient ID No. 

  • Email address 

  • Contact Number 

  • Company Name, 

  • HMO Company 

  • HMO Card No., 

  • Age 

  • Sex 

  • Marital Status 

  • Birthday 

  • Vital Signs 

  • Product 

  • Plan 

  • ID Card No. 

  • Effectivity Date and Expiry Date, 

  • Type of Consultation 

  • Chief Complaint/Initial Diagnosis 

  • Brief Medical History 

  • Pertinent Physical Findings 

  • Final Diagnosis 

  • Medical Clearance 

  • Procedure 

  • X-ray details such as X-ray number, X-ray results 

  • Dashlab ID 

  • Philhealth Details 

 

 

When Do We Collect Your Personal Data? 

 

We may collect your Personal Data in a variety of ways, including, but not limited to, when you: 

 

  • Visit our clinic and avail our services 

  • Register with us by accomplishing our forms 

  • Provide documents like LOA, referral slip, etc. 

  • Communicate with us through our official platforms like email, social media accounts, websites, etc. 

 

 

 

How Do We Use Collected Personal Data? 

HealthFirst collects and uses Personal Data for the following purposes: 

  • Creation of your profile through our system 

  • Provide services and fulfill your requests 

  • Generation of bills/SOA for your payment for the rendered services 

  • Respond to your inquiries, and send updates regarding your requests or complaints 

  • Send a digital copy of your results via email. 

 

 

How Do We Disclose or Share Your Personal Data? 

 

We do not sell your data. We may share it with: 

 

  • Authorized clinic staff and healthcare professionals 

  • Your employer for health program reporting (only as required and agreed) 

  • Third-party service providers under strict confidentiality agreements 

  • When required by law or government authorities 

 

 

How Do We Protect Your Personal Data? 

 

We adopt appropriate data collection, storage and processing practices and have set up adequate organizational, physical and technical security measures to protect the confidentiality, integrity, and availability of your Personal Data and secure against unauthorized access, alteration, fraudulent misuse, disclosure, destruction and/or any other unlawful processing of your Personal Data stored in our systems. 

 

Unfortunately, no data transmission over the Internet or data storage system can be guaranteed to be 100% secure. If you have reason to believe that your interaction with us is no longer secure (for example, if you feel that the security of any account you have with us has been compromised), please immediately notify us of the problem by contacting us in accordance with the “Contact Us” section below. 

 

How Can You Access, Change, or Delete Your Personal Data? 

 

You are entitled to certain rights in relation to the Personal Data collected from you, including the right to access and correct your Personal Data being processed. If you would like to review, correct, update, or delete the Personal Data that you have provided to us, please contact our Data Protection Officer via email at dpo@healthfirst.com.ph. We will try to respond to your request within 24 to 48 hours. 

 

Retention Period 

 

We retain your personal data only for as long as necessary to fulfill the purposes stated above or as required by law (e.g., DOH Circular No. 2021-0226). After this period, data will be securely deleted or anonymized.
